Specifically trained for oncology navigation and care, our multidisciplinary Care Team engages members early in their cancer journey and supports them through treatment and beyond, addressing clinical and non-clinical patient needs in close coordination with their oncologist. 

Our Care Team is available to Thyme Care-eligible members with cancer, making personalized cancer care navigation services more accessible for holistic support in-between visits.

Around-the-clock virtual support from our oncology Care Team is available 24/7 via virtual support to address urgent clinical needs while coordinating with the oncologist.

SDoH-related barriers support

Our oncology Care Team leverages oncology-tailored playbooks, assessments, and in-depth knowledge of local resources to address all SDoH-related barriers that arise.

Cancer and pre-existing condition management

The clinical navigation team proactively mitigates anticipated issues related to pre-existing conditions and coordinates back with the oncologist and PCP.

Palliative/end of life support

Ensure goal concordant cancer treatment at end-of-life through advance care planning and referrals to palliative care and/or hospice, as appropriate, providing cancer care navigation to improve member experience at every stage.
