Please read the following information carefully. You are consenting to conduct business using electronic communications, to receive any document or communication electronically, and to utilize your electronic signature in lieu of signing paper documents. As used in this disclosure, “document(s) or communication(s)” means any agreement, authorization, disclosure, notice, or other document related to your enrollment in Thyme Care services, including but not limited to any document or communication that we are required by law to provide you in writing.
Thyme Care collaborates across the cancer care community to provide personalized cancer care navigation and clinical wraparound support, improving access, experience, and costs. To receive Thyme Care services, we are asking you to agree to the program terms and consent to the electronic delivery of documents or communications. This consent can be provided either online by consenting by clicking the checkbox at the bottom of this form or verbally through a phone call.
E-Sign Act
We are providing you with certain required disclosures and information about our services, products, or accounts. The Federal E-Sign Act allows Thyme Care to provide you with documents and communications in electronic form rather than in written form and allows you to use your electronic signature in lieu of signing a paper document after we have provided you with the following information.
Your Consent to Use Electronic Signature and Electronic Delivery of Documents or Communications
You consent to use your electronic signature to sign any document or communication and to receive electronic delivery of any document or communication. You understand that your electronic signature is legally binding, just as if you had signed a paper document.
Scope of Your Consent
If you consent to electronic disclosures, that consent applies to all Required Information we give you or receive from you in connection with our relationship and the associated notices, disclosures, and other documents.
You agree to print out or download required information when we advise you to do so and keep it for your records. If you are unable to print or download any required information, you may call us and request paper copies. If you need to update your e-mail address or other contact information with us, you may do so by calling us at 201-526-8484 and requesting necessary updates.
Your consent does not mean that we must provide the required information electronically. We may at our option deliver required information on paper. We may also require that certain communications from you be delivered to us on paper at a specified address.
Withdrawing Consent
You have the right to withdraw electronic consent without disrupting Thyme Care services. However, any documents you signed electronically before you withdrew your consent will be considered legally valid and enforceable.
