Refer your member to Thyme Care’s cancer care support services

Use this form to refer a member to Thyme Care if they have been diagnosed with cancer or if cancer is suspected.

After receiving your referral, our Care Team verifies eligibility and contacts the member directly to complete enrollment and better understand their needs.
